Task
Student Directions: Here are some questions for you to answer about how you get along with others. Read each statement and choose the answer that best describes you.
1. When someone asks me how I feel, I can tell them, I feel happy, sad or mad. For example, "if I am picked last for kickball I feel..."
2. I know what I am able to do well. For example, "I am good at hockey. Last week I scored two goals."
3. I know my choices can cause someone else to feel happy or sad. "It makes my brother happy when I let him play with my toys."
4. I will tell someone when I make a mistake. For example, "I did my reading work wrong."
5. When something is hard for me I ask for help. For example, "I can ask my friend to help me learn how to skateboard."
6. When I am playing a game I will wait my turn. For example, "I wait for others to take their turn when we are playing Uno."
7. If others were saying mean things to another classmate I would tell them it is not nice. For example, "When my classmates were making fun of a student, I tell them they aren't being kind."
8. When I am not sure what I need to do, I ask my teacher to help me. For example, "I ask my teacher what I need to do next."
9. When I have a problem with my friends or family, I am willing to try my friend's idea to solve the problem. For example, "At lunch, my friend and I want to sit by the same person. My friend had a good idea to have our friend sit in the middle of us."
10. I listen and follow directions I am given. For example, when my teacher tells me to get ready for lunch, I wait my turn to wash my hands and then line up quietly at the door.
11. I know what it takes to be a good friend. For example, to be a good friend to others, I need to be a good listener and play the games my friend likes to play sometimes.
12. I am friends with people who are different than me in some ways. For example, my best friend and I don't celebrate all the same holidays.
13. I know everyone needs personal space. When I am standing by others, I make sure I am at least one arm's length away from him/her.
14. I can tell how someone is feeling by looking at and listening to them. For example, I knew my friend was happy when I saw my friend skipping down the hall smiling.
15. I know the right way to get attention from adults and students. For example, when I want to share a thought in music, I know I need to raise my hand for the teacher to call on me.
